Superior Dental Care, Inc. (SDC)

GENERAL INFORMATION

The Dental Insurance Plan provides coverage for covered preventive, diagnostic, restorative and orthodontic dental services through a managed dental care plan.  The Dental Insurance Plan is a free-standing option under the PHA Group Benefit Program.  It is possible for a member to participate only in this portion of the overall Program.

 

ENROLLMENT  

All PHA practices are eligible to participate in this program· Eligible employees are defined as PHA members physicians and their employees.· Qualified dependents include spouse and unmarried children.  Unmarried children may be covered from birth to age 19; age 19 to age 24,if the child is a full-time student and is a qualified tax dependent of the employee· Enrollment for new employees of PHA practices - the new employee may enroll the first of the month followingdate of hire.· Exceptions for late enrollment:  Group has just joined PHA or an employee is a newhire.

*Please note that the dental plan monthly premium rates are based on joining through open enrollment or as a result of a qualifying event (new hire, loss of other coverage, etc.).  Late entrants are subject to the annual premium amount payable over the remainder of the year.  Participating PHA practices will be billed by the PHA office.  Premiums will be billed on a monthly basis.

VISION:

EyeMed Vision Care is a discounted vision program through LensCrafters. You automatically receive this benefit when you enroll with Superior Dental Care.  There is no charge for this benefit.
