If you do not qualify for admission, all efforts will be made toward providing information for proper
placement in a system that offers you the services you require.
 How Do I Qualify?
- Age 18 and above
- Have one or more medical conditions requiring
interdisciplinary rehabilitation
- Requires oversight by physician to ensure safe and
effective treatment
- Requires nursing care and supervision 24 hours a day
by trained rehabilitation nurses
- Requires at least 3 hours of therapy daily in at least 2
of the following:
- Occupational Therapy
- Physical Therapy
- Speech Language Pathology
- Primary condition is medically stable
- Has sufficient endurance to participate in a minimum
of 3 hours of therapy daily
- Desire to participate in rehabilitation program
- Reasonable expectation for improvement
- Reasonable expectation for return to the community
