 How do I get copies of my hospital records to my physician?
Please note that we cannot process a records request via e-mail. However, you may call, write or
fax your request for copies of your medical records to be sent to your physician. Please include
the physician's name, address and phone number. Also, include your full name, birth date, social
security number, address, phone number and the specific dates of treatment.

and specific dates of treatment. Kettering Health Network out sources the copying of medical records and you will receive
a bill from Healthport for the copies.
 How much will they charge me?
The State of Ohio HB 508 has set the fees for copies of medical records. They are as follows.
Pages 1 - 10 $2.74 per page
Pages 11 - 50 $.57 per page
Pages 51 - up $.23 per page
Plus the actual cost of postage.
 How long will it take to receive my copies?
Usually a week to ten days.
 What makes it take longer than 10 days to receive?
We do not send copies until the physician has completed all of the required documentation of
your care. This can take up to 30 days after your discharge.
 Can my spouse or family come in a review my record?
Only with your written permission. Kettering Medical Center Network is committed to protecting
your right to privacy and confidentiality.
 How can I get a record of a deceased family member?
If your family member expired at Kettering Medical Center Network, you will need to have
written permission from the executor of the family member's estate. You will also need to
provide proof of executorships. If your family member did not expire at Kettering Health
Network, you will need a copy of the death certificate, written permission from the executor
of the estate and proof of executorships.
 With a Power of Attorney, can I review my families records?
Only if the Power of Attorney includes the right to make medical decisions and obtain copies
of medical information.
 Can you fax my records to anyone I authorize?
We only fax for patient care. With proper authorization, we will mail your records per your request.
